Registration Forms

• Request for Academic Leave of Absence Form 
  • International Students must obtain approval from ISSS.
  • Must obtain both Academic Advisor and Director of Graduate Studies approval 
• Course Audit Permit
  • Complete student information section 
  • Complete course information with instructor approval
  • Submit form to the Graduate College directly via Secure Form Drop-Off
• Withdrawal/Cancellation Form
  • Complete student information and select the correct action (Cancellation or Withdrawal). Student must give reason for leaving.
  • Submit form to stat-grad@illinois.edu for department approval. Department will complete the reason for leaving and give a condition for re-entry if applicable. 
  • Office Staff will submit the form to the next approver - ISSS for international students or directly to the Graduate College for domestic students.
